Patient Information

Everything you need to prepare for your first visit, understand insurance and billing, and feel clear about what happens next.

Your first appointment

Arrive prepared. Leave with a clearer plan.

Your first visit is about understanding what is limiting you, what you want to get back to, and which findings are most relevant to your movement and function.

Your physiotherapist will ask about your current concern, health history and goals, then complete an assessment appropriate to your presentation. You will have an opportunity to ask questions and discuss the recommended next steps before moving forward.

Not sure what to book?

You do not need to choose a treatment technique yourself. Tell us what is bothering you and we can help you identify the appropriate starting point.

Physiotherapist assessing shoulder movement

Helpful to have with you

  • Comfortable clothing that allows the relevant area to move when possible.
  • Your insurance or benefits information if you plan to submit a claim.
  • Any relevant referral, report or imaging summary you already have access to.
  • Questions, concerns and the activities you most want help returning to.

Direct access

You can start by contacting the clinic directly.

In many situations, patients can contact a physiotherapy clinic without first obtaining a physician referral. Your individual extended-health plan or third-party payer may still have its own documentation requirements for reimbursement.

If coverage is important to your decision, confirm your plan requirements before the visit or contact our team and we can explain what information to check.

Insurance & billing

Know what to check before your appointment.

Extended-health coverage varies by insurer and by individual plan. A few minutes of checking in advance can prevent surprises later.

Check your benefits

Confirm whether physiotherapy is covered, whether a referral is required, your per-visit limit and any annual maximum that applies to your plan.

Billing at the clinic

Direct billing is available for eligible plans when the insurer and your policy support it. You remain responsible for any portion not paid by your insurer.

Claims & documentation

If your visit relates to a workplace, motor-vehicle or other third-party claim, contact us before booking so we can confirm what documentation and services are currently available.

Coverage note: MiltonPhysioRehab cannot guarantee insurer reimbursement. Your benefits provider is the best source for confirming eligibility and remaining coverage.

Forms & documents

Bring what is useful. We will help with the rest.

Not every patient needs the same paperwork. Your appointment confirmation or clinic team can let you know if anything specific is required for your visit.

Contact the Clinic
01

New patient details

Be ready to share contact information, your main concern, relevant health history and consent information as part of the intake process.

02

Insurance or claim documents

If a payer has provided forms, claim numbers or special instructions, have those available so the clinic can review what applies.

Relevant reports

Specialist notes, operative reports or imaging summaries can be helpful when they relate to the problem, but do not delay contacting us simply because you do not have them.

Do I need a doctor’s referral for physiotherapy?

Many patients can contact a physiotherapy clinic directly. Your insurance plan or another payer may still require a referral for reimbursement, so check your individual policy if coverage matters.

What should I wear to my appointment?

Wear comfortable clothing that allows you to move and, when practical, gives your physiotherapist access to the area being assessed.

Will my insurance cover physiotherapy?

Coverage depends on your specific benefits plan. Confirm the amount available, visit limits, referral requirements and any direct-billing rules with your insurer.

Do I need imaging before I book?

Not necessarily. If you already have relevant reports or imaging summaries, bring them. If additional medical assessment is appropriate, your physiotherapist can discuss that with you based on the presentation.

How long is the first appointment?

Appointment length can vary by service and clinic scheduling. Confirm the expected length when your appointment is booked so you can plan enough time.

What if I am not sure which service I need?

That is completely fine. Start with your symptoms, limitations and goals rather than a treatment name. Our team can help you choose the appropriate starting point.
